Practical protocol: how we monitor patients on off-label metformin

Sharing what we settled on after two years: baseline B12 and eGFR, repeat B12 annually, eGFR at 3 and 12 months, hold before contrast imaging. We stopped routine lactate testing — it never changed a decision. Curious whether anyone still monitors it.
